Job Title: Home Health Physical Therapist

Position: PHYSICAL THERAPIST

Summary: As a Physical Therapist within our organization, you will play a vital role in improving the quality of life for patients in a home health setting. You will report to the Clinical Supervisor and are responsible for delivering high-quality physical therapy services aligned with a physician's plan of care.

Key Responsibilities:

  • Assess and evaluate the therapeutic, rehabilitative, and functional status of patients.
  • Develop and implement patient-centric plans of care in accordance with state practice guidelines.
  • Evaluate home environments to ensure optimal therapy outcomes.
  • Instruct patients and their families or caregivers on the use of therapeutic appliances and exercises.
  • Monitor and report on patients’ responses to treatment and changes in their conditions to their physicians.
  • Initiate therapy programs and guide other caregivers through significant aspects of the therapy plan.
  • Prepare and submit clinical progress summaries based on the achievement of therapy goals.
  • Engage in discharge planning and consultations, ensuring a smooth transition from therapy services.
  • Supervise physical therapy assistants and evaluate home health aide care when applicable.
  • Maintain the highest level of patient confidentiality and adhere to organizational policies.

Qualifications:

  • A degree in physical therapy from an accredited program.
  • Valid license to practice as a physical therapist in California.
  • A minimum of two years of professional experience; community or home health experience preferred.
  • Strong verbal and written communication skills, along with excellent organizational abilities.
  • Current CPR certification.
  • A valid driver's license with a reliable and insured vehicle per organizational requirements.
